A clinical study upon the incidence of cardiac arrhythmia in thiopental sodium and propanidid during induction






Abstract
To evaluate the incidence of cardiac arrhythmia during induction after thiopental sodium, or propanidid as an induction agent, we studied 100 adult patients who were undertaken elective surgery in ASA class I-II dividing into thiopental group (thiopental 5mg/kg) and propanidid group (propanidid 6mg/kg), and recorded and measured the electrocardiogram and blood pressure before, during and after induction.
The following results were obtained:
1. Heart rate was significantly increased and blood pressure was significantly decreased after thiopental or propanidid intravenous injection and the magnitude of hypotension was more marked in propanidid.
2. Heart rate remained increased after succinylcholine in both groups.
3. Heart rate and blood pressure were significantly and maximally increased after intubation and 1 minute after intubation in both groups. The elevated heart rate persisted for about 10 minutes in thiopental group and 5 minutes in propanidid group but blood pressure returned to control value within 2 minutes after intubation.
4. The incidences of cardiac arrhythmias pre-and post-intubation were 86% in thiopental and 90% in propanidid but they were transient and 94.3% of the total arrhythmias were sinus tachycarida.
There were 4 cases (8%) of ventricular extrasystol only in thiopental group. From the above results, we concluded that the incidence of cardiac arrhythmias and the effects on heart rate¢¥ and blood pressure after intubation in thiopental or propanidid were very similar and the arrhythmias were transient and no need for treatment.
